To take advantage of dispense4me's Prescription Delivery Service, please follow these instructions:
1.Fill out and print off the following Prescription request Form (you will need adobe reader) or webversion
2. Please complete your payment details and contact telephone numbers on the form.
3. Check that the address on your prescription is correct and includes your postcode. If you wish to have your medicines delivered to an address other than the address on your prescription, simply add the details in the appropriate section of the form.
Please be aware when choosing your address that a signature will be required on delivery.
4. Post the completed form ALONG WITH YOUR PRESCRIPTION to us at:

5. Wait for your prescription to be delivered. Please allow at least 5 working days from the date you post your prescription.
